CALDWELL, DR. JOHN D.-Irwin, Pa. Born Nov. 1, 1872. Enlisted in the military service on Nov. 1, 1918. Was assigned to the Medical Corps at Camp Greenleaf and was transferred to New York City where he was the sanitary officer in charge of Debarkation Hospital No. 5 with Headquarters in old Grand Central Palace. Was honorably discharged from service on June 28, 1919, with rank of Captain.
